Acerca del sistema de facturación en los hospitales

Todo indica que el hospital debería facturar y cobrar por las prestaciones que realiza. Pero esto no sucede por diversos motivos que trataremos de analizar.
La determinación de la Superintendencia de Servicios de Salud (S.S.S.) de fiscalizar el cumplimiento de las obligaciones de las Obras Sociales para con los Hospitales Públicos de Autogestión (HPA) a través del sistema débito automático remite a las dificultades históricas que han tenido los hospitales para cobrar (y facturar) por los servicios que brindan. Por medio de este sistema, la AFIP descuenta directamente de la recaudación de la Obra Social el importe correspondiente por los pagos de las prestaciones realizadas a sus beneficiarios.
Todo indica que el hospital debería facturar y cobrar por las prestaciones que realiza. Pero sucede que en muchas ocasiones el mismo personal del hospital no se preocupa por acceder a los datos del beneficiario y éste tampoco da a conocer si tiene o no cobertura, la mayoría de las veces por comodidad.
El sistema de autogestión habilitó a los hospitales a facturar a las obras sociales a través de un nomenclador. El beneficio más claro de la descentralización fue justamente la posibilidad que comenzaron a tener los hospitales adheridos a este sistema para facturar y cobrar por sus servicios, dado que se abrió un registro de HPA donde se les da prioridad para cobrar las prestaciones realizadas a la seguridad social. Incluso de que surja la modalidad del cobro por débito automático y dadas las dificultades, existía ya en muchas provincias alguna normativa habilitante para el recupero de fondos.
Es de tener en cuenta, que el hospital ha visto incrementada enormemente su demanda, en gran parte a raíz de la gran franja poblacional que quedó sin ningún tipo de cobertura en la seguridad social.
Por otro lado, las obras sociales, han utilizado frecuentemente los hospitales para descargar los gastos más elevados en salud, como sucede con los pacientes crónicos.
Luego, los accidentados y las urgencias son una gran puerta de entrada al hospital, generando un costo que debería ser siempre reconocido por la obra social de la persona, abonando la factura correspondiente.
Ahora bien, separemos el tema de la facturación, del cobro efectivo por las prestaciones realizadas, ya que a veces facturar no es garantía de cobro. Pero si no existe un adecuado sistema de facturación seguramente no se va a cobrar. Ahí es donde deberíamos tomar conciencia de lo importante que es medir en salud. Es el punto de partida llegado el momento de desarrollar políticas sanitarias y mejorar la distribución de recursos, además de ayudar a mejorar la calidad de las prestaciones.
La situación es de tal precariedad, que algunos hospitales ni siquiera desarrollaron un “sistema de facturación” ya que no reciben casi pacientes con cobertura.
Un informe de auditoria de la SIGEN, detectó serios déficits en los procedimientos de facturación de un hospital de la Pcia. de Bs.As. (perteneciente al primer cordón bonaerense): los procedimientos para la facturación no estaban normatizados, no se cumplía correctamente con el trámite de admisión, se detectó un 30 % de TAC y estudios de Resonancia Magnética Nuclear sin facturar, en algunas facturas emitidas no figuraban los datos de los pacientes atendidos, existían dificultades en las áreas para la detección de pacientes con cobertura médica, el sistema informático no permitía consultar el padrón de la Obra Social, etc..
Es posible que no se sepa cuantos pacientes necesitaron realizarse una T.A.C. en determinado hospital? Es acaso un dato “menor”?
En algunos hospitales se comenzó a tomar conciencia respecto de los ingresos a los que renunciaban, en gran parte por desidia. Varios hospitales de la Pcia de Bs.As, actualmente le dan gran importancia a la tarea de “admisión” donde se les exige a los pacientes documentación personal, y en el momento se determina si tiene aportes, y como consecuencia cobertura social. Además se incorporaron bases de datos con los padrones de las principales obras sociales.
Los empleados a su vez tomaron conciencia de que las mejoras en la facturación los beneficiaba a ellos (mejora su salario por lo recaudado a través del SAMO).
Más importante aún, es que los ciudadanos, asuman y declaren que tienen obra social, colaborando así para mejorar su propia atención.
Como resultado de estos cambios, en algunos hospitales, la facturación aumentó notablemente.
Ahora bien, cuanto perdía el sistema de salud pública por poseer un deficiente sistema de facturación?
Cuánto se puede mejorar aún?
No es un tema menor, se trata del método para recaudar los fondos que ayudarán a sobrellevar los costos de funcionamiento, la adquisición de insumos hospitalarios y medicamentos para brindar asistencia a los sectores más desprotegidos de la sociedad.
