Historia Clínica y Auditoría

Al ser, la historia clínica, el primer contacto con la realidad, la Auditoría tiene la posibilidad de introducirse en el proceso para predecir futuros conflictos y anticipar su solución.

Podemos definir la Auditoría en sistemas de salud como un instrumento de evaluación de la eficacia, eficiencia y efectividad de los procedimientos de atención médica.

Se basa, en primer lugar, en la revisión sistemática, continua y objetiva del trabajo profesional médico teniendo como base, el análisis de las historias clínicas.

¿Por qué las historias clínicas?

Porque la historia clínica constituye un instrumento fundamental en el desarrollo de las siguientes funciones:

Contribuir con la asistencia y/o atención de salud de calidad.
Favorecer la docencia y formación continua de los recursos humanos
en salud.

Planificar, organizar, gestionar y evaluar la actividad asistencial de los
diferentes servicios y unidades del establecimiento de salud.

Contribuir al discernimiento de aspectos jurídico-legales relacionados
con la atención de salud.

Contribuir a aportar datos epidemiológicos.

Al ser, la historia clínica, el primer contacto con la realidad, la Auditoría tiene la posibilidad de introducirse en el proceso para predecir futuros conflictos y anticipar su solución. Al mismo tiempo, de sus evaluaciones se puede obtener información estratégica, a fin de extraer conclusiones, tendencias y/o desvíos.

Por otra parte, la Auditoría médica debe bregar para que la historia clínica refleje la totalidad de la información y los datos, porque de la calidad de su contenido dependerá la calidad de sus conclusiones.

La historia clínica constituye una fuente de datos insustituible, es un instrumento legal y su contenido médico posee un alto valor. De la historia clínica podemos extraer datos de distinta índole como administrativos, datos del paciente e insumos, medicamentos, etc., con los cuales podemos elaborar información de costos, que nos permita actuar sobre la realidad económica que hay que administrar.

Si la gestión evalúa los costos de una atención sólo por la patología principal, sin considerar la historia clínica, se puede inducir a errores, al despreciar el origen del desvío que puede deberse, entre otras causas, a una complicación de la misma patología, un agravamiento por enfermedades preexistentes no relacionadas con la patología principal, una infección intrahospitalaria o una inadecuada utilización de recursos

La auditoría es una revisión de la práctica médica con carácter educativo.

Tiene tres objetivos:

Conocer la calidad de los servicios que se prestan.
Formular proyectos y sugerencias para mejorarlos.
Conocer las condiciones en que se desarrolla el trabajo profesional.

La Auditoría se puede llevar acabo con el enfermo internado, o bien luego de producido el egreso. La Auditoría médica, es en última instancia, un mecanismo de mejoramiento y perfeccionamiento médico, basado en la utilización de normas y registros. De esta manera, es concebida como un procedimiento técnico de control, con un componente docente y facilitador de estímulos; nunca se ha concebido como una oficina creada para la recepción de quejas y reclamos.

El sistema de evaluación médica debe cumplir con los siguientes objetivos:

Conocer la calidad de los servicios y valorar los factores que inciden sobre la buena marcha de los mismos.
Analizar el trabajo médico en sus diferentes áreas, con el propósito de conseguir el más alto nivel de eficiencia en la atención prestada.
Propender por la creación de estímulos y facilitar la información para la asignación de los mismos.
Coordinar los aspectos referentes a la investigación, con el objeto de
lograr una proyección médica más amplia.
Establecer los criterios para programar la capacitación científica.
Estimular el conocimiento y las buenas relaciones entre los profesionales de la salud.
Dar cumplimiento a las disposiciones legales.

La acción correctiva en Auditoría

Es toda acción que, en el marco de la mejora continua de la calidad del servicio de salud, apunta a corregir una no-conformidad específica. Es posible que una no-conformidad no pueda ser revertida en el corto plazo; sin embargo, la acción correctiva contempla la verificación periódica de los avances por revertir la no-conformidad.

Toda acción correctiva deberá ser planificada y programada en función a la envergadura de la no-conformidad, estableciéndose los plazos e indicadores necesarios a los fines de revertir anomalías.

La acción preventiva en Auditoría

Es toda acción que, en el marco de la mejora continua de la calidad de servicio de salud tiene como objetivo plantear uno o varios mecanismos alternativos para evitar que una no-conformidad hallada se vuelva a presentar en lo sucesivo.

La acción preventiva será definida de manera participativa y colaborativa entre el auditor y el responsable del servicio auditado.

Las conclusiones de la Auditoría

Contemplan principalmente los hallazgos más importantes, los plazos para rectificaciones y los compromisos del sistema auditado para prevenir la recurrencia de las no-conformidades detectadas.

Debe traducir exactamente la información que desea encontrar.

Debe permitir evidenciar una circunstancia prevista.

Debe ser transparente y clara en cuanto a su intencionalidad de investigación.

La Auditoría debe ser hecha con patrones preestablecidos, de modo que pueda ser independiente de la percepción personal de la persona que audita.

Debe ser objetiva, no subjetiva, de tal manera que el patrón utilizado podrá ser aplicado por cualquier auditor y obtener el mismo resultado.

Requisitos para la Auditoría

Existencia de criterios explícitos e indicadores de los productos.

Equipos de salud capacitados para definir por consenso los productos
esperados y sus requisitos de calidad.

Contar con los instrumentos para recoger la información.

Con respecto a las características que deberían tener las historias clínicas, debería determinarse un modelo único, estandarizado y obligatorio. De esta forma, en todos los centros asistenciales del país, las historias clínicas contendrían la información con un mismo ordenamiento y con un esquema y definición predeterminados de los datos a completar.
