Una visión del rol de la Auditoria en el estado actual de cosas

Los siguientes párrafos están tomados de la monografía “Costos en la atención de la salud” presentada para el CURSO ANUAL DE AUDITORIA DEL HOSPITAL ALEMÁN año 2006. La misma está disponible en la biblioteca del Hospital Alemán
El auditor médico o asesor médico, cumple distintas funciones en la práctica cotidiana que no necesariamente refieren al área de auditoría, entendida como comparación con las normas o con una referencia definida del sector al que pertenece, sino que cada vez más se ve involucrado en tareas de gestión, negociación, presupuestos, etc. Que implican que el auditor tenga conocimientos de distintas disciplinas y las aplique en forma correcta y conveniente para mantener una buena relación tanto con el financiador o el prestador indistintamente de donde desempeñe su función.
En este contexto y con la transformación de los conocimientos se generan cambios en el sistema como así en la fuerza laboral en que está inmerso. De esta manera vemos la confluencia de intereses entre la medicina entendida como salud pública, la economía de la salud, la tecnología aplicada y atención personalizada del paciente en un ámbito de seguridad y calidad.
El auditor requiere de un marco de referencia, que obligatoriamente es multidisciplinario. Consideramos desde un comienzo y como línea de pensamiento unívoco que el valor defendido, independientemente del contexto en que se desarrolle la función del auditor se da en el marco de la bioética moderna. El auditor médico se ve enfrentado en la práctica cotidiana a nuevas reflexiones frente a desafíos sobre los cuales tiene que tomar posición muchas veces influido por presiones externas ajenas y con grandes dudas sobre los beneficios que acarrearía determinada práctica o el ejercicio de determinados derechos que no gozan de consenso en la comunidad médica. Ejemplo de estos son: procedimientos para extender la vida, la relación médico paciente, la prescripción de tratamientos, avances tecnológicos sin consenso sobre efectividad, tecnología de la reproducción, etc., sin olvidar también los aspectos ligados a la información y su privacidad (derecho de las personas) el cual se ve muchas veces violado en pos de la eficiencia administrativa y aquellos temas en donde están involucrados las instituciones de salud o la salud pública.
También es de destacar la importancia que tienen este concepto la capacidad del auditor para involucrarse en los profundos y vertiginosos cambios del entorno y cómo jugar como partícipe y testigo de la transformación, que implica una toma de posición respecto de temas que lo afectan en su función: como el ensanchamiento de la brecha económica, el rápido crecimiento de la población, el recrudecimiento de enfermedades infecciosas. Y la aparición o reactivación entre otras de condiciones sociales, habitacionales, y ecológicas cada vez más degradadas.
Independientemente del enfoque y su aplicación, prevalece la persona como ser único e irrepetible, y si bien los intereses públicos o comunitarios pueden estar enfrentados con los individuales, debe prevalecer el enfoque ético, la moral comunitaria, y la posibilidad de solicitar ayuda al momento de la toma de decisión.
