Oftalmología: Tecnología a la vista

En San Juan están previstos nuevos métodos de diagnóstico para tratar problemas de visión.

Dentro del ojo hay una lente de aumento llamada cristalino. Con el pasar del tiempo o distintas enfermedades pierde su transparencia. A este estado se le denomina catarata. Para tratar esta y otras afecciones en la visión, está previsto que en los próximos días llegue a San Juan, nueva tecnología para hacer un diagnóstico más preciso y con un mínimo margen de error. Así lo afirma el oftalmólogo Reinaldo Echavarría (MP 10.5827), quien será encargado de presentar esta novedad en la Clínica Del Prado, de San Juan.

El profesional es especialista en cirugía oftalmológica y actualmente es cirujano de referencia de cuatro centros de salud de Buenos Aires: Clínica de Microcirugía, Centro Integral de Medicina Ocular, Clínica de Ojos Escobar y Clínica Modelo Los Cedros.

Echavarría cuenta que el equipo que funcionará en la provincia se llama “Iol Master”. Se usa para obtener, mediante interferometría láser, la medida de la lente que va a ser implantada dentro del ojo durante la cirugía de catarata. Es decir que mediante una luz láser se obtiene el largo axil del ojo con un error estadísticamente no significativo; estos sumado a las nuevas fórmulas de avanzadas (incorporadas en el equipo) permite a los cirujanos oftalmológicos predecir esta medida a tener en cuenta en el momento previo a la cirugía de cataratas.

Por otro lado se trata de un estudio ideal para realizar en aquellos pacientes con ojos demasiado cortos o demasiado largos, o sea con hipermetropía elevada, miopía elevada, astigmatismo, pacientes con antecedentes de cirugía de retina, queratocono o cirugía refractiva previa, siendo estos los casos mas complejos para poder calcular la graduación de la lente intraocular.

Se trata de un estudio altamente probado y reconocido a nivel mundial que debido a la importancia del mismo, a partir de su llegada a la provincia no será limitado a uso privado únicamente. El mismo será cedido también a cualquier cirujano que desee utilizarlo y a cualquier paciente que requiera su uso, agrega Echavarría.

Esta nueva tecnología estará instalada en la Clínica del Prado, a partir de la inauguración de su Servicio de Oftalmología, que está prevista para el día 13 de noviembre próximo.

La exactitud de la medida de la lente que va dentro del ojo tiene dos ventajas fundamentales: la primera es que el paciente en el momento previo a la cirugía podrá optar por cual lente se implantará en su ojo. Si la elección es la de un lente monofocal (una distancia) lo acercará a una condición ideal teniendo un 90 por ciento de probabilidades de no dependencia de anteojos posterior a la cirugía, salvo que el paciente presente defectos agregados preexistentes como astigmatismo elevado o alguna enfermedad en la córnea como el queratocono. De esta forma sólo requerirá corrección de cerca. (lectura, computadora, etc).

La segunda ventaja es que los cirujanos de catarata podrán ofrecerle a los pacientes pacientes el implante de las nuevas lentes intraoculares de ultima generación.

Presbicia

Para enfocar de cerca el ojo pone en marcha un mecanismo llamado “acomodación”. Esto se logra mediante la acción del músculo ciliar que varía el espesor del cristalino. Con el paso del tiempo este músculo comienza a debilitarse y el cristalino comienza a endurecerse causando un estado fisiológico y no patológico, llamado presbicia. O sea, es un proceso natural y no una enfermedad.

Se manifiesta a partir de los 40 años y sus síntomas iniciales son muy claros: hace falta separarse de lo que estamos leyendo, o de la tarea que estamos realizando y se necesita cada vez más luz para realizar la misma.

Actualmente no hay ninguna cirugía eficaz para tratar la presbicia directamente, dice Echavarría. Agrega que, el paciente que por indicación médica exclusivamente va a ser intervenido de una cirugía de catarata puede optar al momento previo de la misma elegir un implante multifocal y de esta forma poder prescindir del uso de anteojos tanto de lejos como de cerca logrando así una visión muy buena de lejos (más de 3 metros), aceptable de intermedia (computadora) y muy buena de cerca (lectura).
